Attention all Coaches, Managers, Trainers, and Volunteers  
If you have not already done so please fill out thepolicecheck12 form, scan, and return to me before you do anything else.


Police checks are required for all volunteers in the association this is mandated by WMHA and the OMHA.  

Please remember that this is for the protection of the children in our association.  

If you or a staff member have not completed a form for the 2012/13 season one is required by Friday Nov 23. 2012. 

If I have not received one for you by Nov 23, 2012 please expect a phone call on Nov 24th.
